Overview

Laminitis is a painful and potentially devastating disorder of the suspensory apparatus of the distal phalanx within the hoof. Failure or weakening of the lamellar interface can allow displacement or rotation of the distal phalanx and can produce lasting structural damage. Laminitis is a syndrome rather than a single disease, and the initiating process may be endocrine, inflammatory, mechanical or associated with systemic illness.

Early recognition and prompt veterinary management are critical. A horse with suspected acute laminitis should be treated as an urgent case even when radiographic displacement is not yet apparent. Management aims to reduce pain, limit further lamellar injury, support the foot, identify and treat the trigger, and monitor for structural change.

Clinical significance

The consequences range from transient foot pain to chronic lameness, hoof deformation, recurrent abscessation, distal phalanx displacement and euthanasia. The disease can affect one or more feet, with the forefeet commonly involved. Some horses show dramatic reluctance to move, while others have subtler changes such as shortened stride, frequent weight shifting or unusual turning behaviour.

Endocrine-associated laminitis is particularly important because affected horses and ponies may remain at risk after the acute episode unless insulin dysregulation, pituitary pars intermedia dysfunction, obesity and pasture exposure are addressed. Laminitis associated with sepsis, colitis, retained fetal membranes or carbohydrate overload may progress rapidly and requires simultaneous treatment of the systemic disease.

Aetiology and risk factors

Major categories include endocrinopathic laminitis, sepsis-associated laminitis, supporting-limb laminitis and laminitis following excessive carbohydrate exposure. Endocrine risk is linked to insulin dysregulation and may occur in equine metabolic syndrome or pituitary pars intermedia dysfunction. Obesity, regional adiposity, previous laminitis and access to high non-structural-carbohydrate pasture or feed increase concern.

Supporting-limb laminitis may develop when severe lameness in one limb causes prolonged overload of another. Systemic inflammatory disease, especially gastrointestinal or reproductive disease, can trigger lamellar injury. Risk assessment should therefore extend beyond the feet to diet, body condition, endocrine history, recent illness, medication, pasture change and contralateral limb disease.

Clinical presentation

Signs include reluctance to walk, a short or pottery gait, difficulty turning, shifting weight, increased time recumbent, heat in the hoof wall and increased digital pulses. The classic stance with the forelimbs placed forward is not always present. Pain may be worse on a firm surface or during tight turns. Chronic cases may show divergent hoof rings, a widened white line, altered hoof shape, dropped sole or recurrent solar abscessation.

Examination should be calm and deliberate. Observe stance, willingness to move, turning, foot placement and weight distribution. Palpate digital pulses and assess hoof temperature in context, recognising that neither finding is specific alone. Hoof-tester response can help localise pain but may be variable. Record baseline findings so progression or response can be judged objectively.

Diagnostic approach

Diagnosis is primarily clinical, supported by imaging and investigation of the underlying cause. Lateromedial radiographs provide a baseline for distal phalanx position, hoof-wall relationship, sole depth and future comparison. Marking the dorsal hoof wall and identifying the coronary band or apex can improve interpretation. Additional views may be useful depending on chronicity and complications.

Endocrine testing should be considered when signalment, body condition, history or season suggests insulin dysregulation or pituitary disease. Testing strategy depends on whether the horse is acutely painful and on local laboratory guidance. When systemic disease is suspected, investigate the primary disorder promptly. Venography, advanced imaging or specialist referral may assist selected complex cases.

Differential diagnoses

Differentials for acute bilateral foot pain include solar bruising, excessive trimming, thin soles, bilateral abscessation, pedal osteitis, navicular-region pain and other causes of multifocal lameness. Unilateral severe foot pain may be caused by an abscess, fracture, septic process or penetrating injury. The history, distribution of pain, digital pulses, hoof testing and imaging help distinguish these conditions.

Treatment and management

Initial management should minimise movement and mechanical stress. Deep supportive bedding and immediate frog or sole support may improve comfort and reduce damaging forces, but support must be individualised and repeatedly checked. Forced walking to “improve circulation” is inappropriate in an acutely painful horse. Transport should be avoided unless necessary for care and undertaken with suitable support.

Analgesia and anti-inflammatory treatment are selected by the attending veterinarian according to severity, hydration, gastrointestinal and renal risk, and concurrent medication. Pain that remains severe despite initial treatment warrants reassessment and often specialist input. Cryotherapy is used in selected at-risk or early sepsis-associated cases where practical, with technique and duration managed carefully.

The initiating disease must be addressed. This may involve treatment of colitis, metritis, retained fetal membranes, pneumonia or other systemic illness; reduction of dietary non-structural carbohydrate; management of insulin dysregulation; or protection of an overloaded supporting limb. Nutritional management should provide appropriate forage while avoiding abrupt starvation. Farriery and hoof-care decisions are guided by examination, comfort and imaging, with close communication between veterinarian and farrier. The goal is to improve load distribution and reduce stress on damaged lamellae without creating new pressure injury.

Monitoring and follow-up

Monitor comfort, stance, willingness to move, digital pulses, hoof temperature, appetite, recumbency and adverse effects of treatment. Repeated radiographs may be needed to identify displacement and guide hoof care. Frequency depends on severity and progression. Support devices and bedding should be inspected for pressure points, movement and contamination.

Long-term follow-up includes hoof growth, sole depth, white-line integrity, recurrent abscesses, body condition and control of the underlying endocrine or systemic risk. Return to exercise is gradual and based on comfort, hoof stability, imaging and the responsible clinician’s assessment rather than a fixed timetable.

Prognosis

Prognosis varies widely. Horses without displacement that respond promptly and have a controllable trigger may return to useful function. Prognosis becomes more guarded with severe or persistent pain, progressive distal phalanx displacement, inadequate sole depth, solar penetration, infection, multiple affected feet or uncontrolled endocrine disease.

Honest communication is important because prolonged treatment can be intensive and costly. Quality of life, ability to rest, response to analgesia and the realistic possibility of maintaining a comfortable foot should guide decisions.

Prevention

Prevention focuses on risk recognition. Maintain appropriate body condition, introduce dietary changes gradually, control access to high-risk pasture where indicated and investigate previous laminitis or regional adiposity for endocrine disease. Horses with pituitary disease or insulin dysregulation require ongoing management rather than a one-time intervention.

Prompt treatment of systemic inflammatory disease, careful management of retained fetal membranes and active protection of the supporting limb in severe unilateral lameness are also important. Owners and yard staff should know early warning signs and contact a veterinarian before forcing exercise or making major dietary changes.
